Questionnaire form improvements
Provide more control over spacing between lines, margins, fonts, page breaks. A WYSIWYG interface, similar to creating a form in Adobe Acrobat or file in Word, would be better. Allow the questionnaire to be condensed and not be as many pages. [155013 154691 161187]

COME ON DENTRIX!!! No response to this. Listen to you users. Dentrix has some great ideas and tools, but when it comes to practical use in the dental office they are useless. Questionnaires as currently formated is one of them. Greater control over the format of the questionaires and better ability to review and update health histories would be great. All of this as stated previously by other users. Please listen to your users and get this taken care of!!!

I would like to be able to do the following in the questionnaire module:
1. Decrease the amount of pages, by moving Questionnaire lines closer together so that less white space is seen on the form.
2. Split up individual fields for "Patient Info" for ALL, BRIEF, and Partial versions of Patient Information linked questions so that you can break up each field as needed to include/exclude on the field.
3. Allow more than one question to be placed on the same line.
4. Allow the graphics to be changed for the Forms so a letter head could be imported for the office's letter head.
5. Allow the size of each question field to be sized to desired ratio or font type.
6. Allow a list of customized "linked fields such as check list questions" to be linked directly with some new window in the family file or some other modules as a"quick form list window" that would be able to be quickly see the results of the check lists the office commonly uses at a glance.
7. Allow the office to move questions around more dynamically, currently the lists become very large format and is difficult to word the questions.All of the above, please!!! The concept of the online questionaire and kiosk is great...needs some revising, though...

The questionaire needs a lot of work. This is definetly one of them. Saving changes on an updated questionaire seems impossible.

This should have been fixed years ago! You cannot expect staff to want to go through 13 pages of Health Hx, when it could show up on a single screen. Please Make This Happen!!!!!

there has to be a way to condense the form in smaller font for viewing purposes.. Too many pages to scroll thru to get the necessary information.

Need to create a way when health histories are reviewed and updated at each visit to maintain a copy of the orginal but then make the new changes right on the present questionaire form. If you change it now there is no record of what the health circumstances were 6 months prior etc. If the question comes up later if they were on blood thinners at the time of previous appt there is no real way of knowing if you go in and make changes and that change was possibly on blood thinners now.

We would actually use the questionnaire system if they would not be so clunky! As it is, we've had to find outside, secure, third party online forms for our patients to complete that only end up being two or three pages long, but with Dentrix's modules in the questionnaire fields (even if we customize them) our paperwork is about 6 pages long. Too much!

We need the ability to update or expand on medical histories for patients with extensive medical problems and medications. The form we use is 5 pages, but when you have patients with multiple illnesses or diseases, I would like to see an expansion of questions that is prompted by checking certain illnesses. Patients complain about 5 pages now, but would like an area that we can get more information from them.

My 6 page in-office questionnaire is now 17 pages online, and I get complaints every day it is too long. I am told today that the timeout is only 15 minutes. It should be 30 minutes to allow for interruptions so patients do not need to start over again. We lose credibility and patients when this happens.

I love Dentrix and have used it for over 15 years, the ecentral, questionaire module is the worst! Did you even take into account what happens in adental office, signatures are ridiculous, patients think I'm trying to get over on them by signing later. its quicker to look at paper forms then traverse through each module to find out whats going on with a patient. Definitely need to work on this, or you may lose a lot of users. Other systems in offices I temp at have the ability to do everything we are asking you to do so it is capable.

I am only using this right now due to the cost. We are not happy with the layout at all. Too much wasted space. And I agree, one question should be able to prompt another. Ie: Do you have dental insurance? Yes and a new field pops up to add the policy information. Dentrix is having seminars, webinars, and articles published about technology making processes easier for dental offices, but this one is definitely a burden right now.

Please note that there are many topics relating to how questionaires are awkward and with regard to medical history that can put a practice at high risk. We need simpler pdf like forms and easy to pop-up access and place to add comments and have the patient sign them. LET Steve Jobs be your guide to designing this and you will win over so many users.

Dentrix, why are overlooking the complaints of your clients? Why have you not looked at the technology of third parties and incorporated them into the programs? Another system of evaluation should be incorporated into your rationale for altering the program. It should be based on the number of votes the clients render. Some concepts may not get a high number of votes but there are very pertinent to your client base. Some clients may not have written any comments or have voted. These clients may have opted to leave Dentrix because issues were not being addressed. All comments should be reviewed not just the one the computer ranks high. You have quite a few dissatified clients. It does not have continue. Listen to your clients.

This is the WORST module ever created in/by Dentrix. With Prod pack 8 downloads our 3 page Med Hx became unformated and now 4 pages. We were told it was from astericks added to questionaires as required fields. I saw none on our Med Hx form so clicked edit to see them on the set-up. Now I have a new form date and we have lost access to all the previous years medical history as far as Enter New Responses pulling the info forward. That means another 15 minutes per existing patient to get back to where we were prior to the Prod Pack download and my trying to see who or why the form changed. It took six months to get the Med HX updated on existing patients and we are NOW starting over again. None of the old Med Hx except Patient alerts will auto populate. All I need is a programmer to reactivate my previous Med HX and am told it is not possible. We also lose the Insured's birthday on every Questionaire since Prod Pack 8.

User should be able to select which patient information fields from the questionnaire will be used to update Dentrix family file. If the user typed their address in all lower case, the user doesn't want to overlay the current, correctly formatted information in the family file.

Additionally, there should also be completely customizable control over Patient Information as well. If all I want on a specific form is the patients is the patient's date of birth, then I should be able to enter a date-based field and define which patient information it will update (without having a bunch of other Patient Information data needed as part of the same request). There are currently preset ones, but if there was more control, that would be very useful. For example, if I only need the SS# or DL# on a specific form, I could create a field that will update that field in the Family File without having to get the address or names or other patient info (as that info would have been collected in a different form). I just wanted to get that in there for consideration when the next update is made.
