  we created a custom form/questionaire for this.

  we do this in patient alerts..... we creat 3 alerts 1. med list 2. dental note- things like don't tip back too far and 3. personal notes - has son in iraq etc

  In the patient chart, maybe above or below the teeth numbers, a box where you can TYPE in notes. We would like to place specific requests of our patients, like pillow requested, or back pain (do not recline) or things of this type.

  Use the patient note in the family file --that is what it is there for. Works great.

  "Chart notations" should have a Text option that could be made to stay on the Chart even when line drawings are deleted or hidden.

  a -peg board type in area right of buttons in pt chart that can be notes put and viewed all time chart is open without any dates or notes open.

  This should be at the top of the list for any practice who cares about making an impression of being a "caring" group. Nothing is more important to a patient's sense of belonging in a practice than having the people providing the care know who they are and to be able to have a conversation about the important things in their patient's life.
