Provide more control over spacing between lines, margins, fonts, page breaks. A WYSIWYG interface, similar to creating a form in Adobe Acrobat or file in Word, would be better. Allow the questionnaire to be condensed and not be as many pages. [155013 154691 161187]

We use questionnaires to update medical history on patient but they don't like filling out their meds or emergency contacts every time they update the form, many say there hasn't been any changes since the last time they were in, so if there was a way to automatically 'import' that info into their new update form it would be eaiser for them to do.

Medications change and x-rays need to be tracked easily to see when the last complete series/panoramic/bitewing series was completed. Could x-rays completed also go directle to the more info button? Medications would also be handy to access quickly with a way to record the changes and dates that go with it.

We are a pediatric office and you have to create a file first with responsible party and then download pt. information into that family file then there are usually 2 parents. Also insurance phone numbers and employers are not imported.

We would like to be able to attach questionaires to emails to forward to specialists or patient.

It would be nice to be able to review and change questionnaires (or have areas for doctor notes) using the iPad chairside so I don't have to stand up behind the patient at the computer while interviewing.
Also allow signature using your finger or a stylus while filling them out on the iPad kiosk.

Once a Questionnaire is signed...you can't change it- for very good reasons. BUT sometimes you really need to add a comment after it is signed...for example medical histories are only updated ever 3-5 years, but a patient may change meds or have something new come up. Being able to add comments to the bottom with a separate signature and date would be helpful.

If a person answers "yes" to having insurance or a change of health history, bring up the insurance questionaire or health history. If they answer "no" they wouldn't have those sections or questionaires to fill out.

Please make it easier to import Health Histories, Hipaa and consent forms from the web site and update them and have patient sign them, there are so many steps right now its too difficult and time consuming

Since information in the Family File could be the correct version (we have patients type in misinformation when filling out forms on the website), to be able to update the information in the questionnaire would be useful, even it that means starting a new questionnaire for the patient. That way the information is correct when we use that same questionnaire form for the update with "new responses" in the future.

Allow offices to use a blank form with no pre-selected header or allow us to upload our company logo as the header. Thanks!!

When updating Pt medications on the questionnaire, a spell check would be helpful. Include generics and PDR info.

Improve HIPAA comliance with online forms and patient accounts

We need a required field created that will allow the patient to fill out their birthday and social security number so that it will go directly into their Family File when the questionnaire is synched. Right now we have to view their questionnaire to find out their birthday & SS# and manually enter it into the Family File.

I would like to be able to save my signature so that when I need to sign as a witness to the patients consent form (which is a lot) I can simply click on my name and my signature shows up along with my typed out name.

After the DDX SP 8 update the default date at the bottom of the questionnaire no longer defaults to that current day's date. It defaults to the date the last time the questionnaire was submitted. It was great before because we could submit and have the patient sign consent for treatment form at each visit with different procedures and the date would automaticaly update, this is no longer the case, it now must be manualy done allowing much more opportunity for error.

No header/footer needed on Questionnaire edit/new response screen. Health hx designed to fit on Kiosk screen without scrolling. To save time (since patient can only sign in Questionnaire module & no need to send kiosk responses to Dentrix server and back here) we have the patient enter directly in the Questionnaire module. Every page scrolls, because the header & footer are displayed. Only display header/footer if questionnaire is printed to paper/pdf/file.
